Incident Reports

Anyone is welcome to submit incident reports. An incident report can be used to document a wide variety of incidents.  Most often, these forms are used when an individual or a group infringes upon campus policies, norms, or expectations.  The Office of Residential Life reads all submitted reports and addresses them appropriately.

Residential Life encourages complainants to supply their names and contact information so that the office may contact you with any follow-up questions.  When submitting a form, you may speak to a member of the staff to discuss the process.
