Below is a list of documents that are frequently requested from students in relation to their financial aid eligibility; however, not every student will be asked to complete these documents.

24-25 Request for Financial Assistance and/or Reconsideration of Financial Aid
**For 2024-2025 school year only** Form used when a student or family would like the financial aid package to be reconsidered due to extenuating or changing circumstances. -
FERPA Student Authorization to Release Financial Records Form
Students who wish to allow our office to speak with a parent, guardian, mentor, or other family members must complete and submit this form. This form is for financial records only and is used by both the Financial Aid and Student Billing Offices. -
Satisfactory Academic Progress Appeal Form
Students who have received a notice of their financial aid suspension may use this form to submit an appeal to be reconsidered if the student experienced extenuating circumstances that impacted their ability to meet our academic requirement.
What else could be missing?
The most common “missing” item is a student’s acceptance or request to decline a student loan. Students who wish to decline the loan offer(s) should send their request in writing (an email is fine) to the financial aid office. Students who wish to accept their student loans may have to complete additional steps. Please check your application portal/award letter or Beloit Portal for additional guidance. You are also welcome to contact the Financial Aid Office for further assistance.
