Without a doubt, the financial aid process is information and paper intensive! In order to comply with federal and state regulations, we often have to request additional information from the student or parents. Or, perhaps, the family has asked that we consider unusual financial circumstances which the FAFSA information does not accurately reflect. Whatever the scenario, you can bet that there is a form involved.

This page contains links to those forms most commonly used in the financial aid process, as well as forms pertinent to student employment. Please note, however, that not all of these forms are required of every financial aid applicant. All financial aid applicants - whether prospective students or current students - must complete the FAFSA and the appropriate Beloit College Financial Aid Application below. If our office needs additional information to complete the financial aid analysis, we will inform the student, either by mail or email, and we will direct the student to the appropriate form, which may be downloaded here.

Forms Related to Financial Aid Applications:

Beloit College Financial Aid Application - This form is required of all domestic students (new and continuing) who are applying for need-based aid.  In addition, you should also be completing the 2013-14 FAFSA, available after January 1, 2013.

Verification Worksheets - For a variety of reasons a student’s FAFSA may be selected by the federal government for a process called verification.  The Financial Aid Office will request additional information in order to evaluate your aid eligibility; no aid will be disbursed until the process is complete.  There are Verification Worksheets (labeled V1-V5) specific to each student and their status (dependent or independent) in addition to other information that may be required.

Off campus study budget worksheet - If you intend to study off campus for either semester (or both) during the 2013-14 academic year, please complete and return this worksheet to the financial aid office electronically (faoffice@beloit.edu).

Financial Aid Appeal Form -  Beloit College uses the FAFSA and the Beloit College Financial Aid Application to determine aid eligibility in a consistent way between all families; however, occasionally special circumstances exist that are not represented by the FAFSA or on the Beloit form, and additional explanation is necessary. The financial aid revision process exists so that Beloit College can continue to be an affordable choice for your family if your circumstances change suddenly, or if new information becomes available that impacts your financial aid eligibility. Please complete and submit this form if you would like the Financial Aid Office to re-evaluate your aid based on new information.

Forms Related to Student Employment on Campus:

Beloit College Student Employment Handbook (2012-13) - This is a guide for work-study employees. A student receives a hard copy of the Employment Guide upon job placement at the College. In addition to summarizing the policies and procedures that govern campus employment, the guide details the payroll dates and touches on a number of other topics pertinent to student employment.

Guidelines for Supervisors - Supervisor responsibilities, payroll procedures, and evaluations are among the topics covered in this guide for employees who supervise student workers.

Employment Opportunity Form (MS Word format) - Offices on campus use this to inform the Financial Aid Office of employment opportunities for students.

Employment Opportunity Form (PDF format) - This is an Adobe Acrobat version of the form above. (Please note: If you have Adobe Acrobat Reader, you can use the "tab" key to move from field to field to fill in the form on your computer, and then you can send it to your printer. However, you cannot save the completed form unless you have the full version of Adobe Acrobat.)

Student Wage Scale - This form details the hourly wages paid for a variety of on-campus jobs.

Wisconsin WT-4 Form - This form is used to determine the amount of Wisconsin state income tax to withhold from a student's paycheck.

W-22 Reciprocity Declaration - Student employees who are legal residents of Illinois, Indiana, Kentucky and Michigan should complete this form. This authorizes Beloit College to exempt qualified students from the withholding of Wisconsin state income tax.

Illinois IL W-4 Form - Student employees who are legal residents of Illinois should complete this form, which is used to determine the amount of Illinois state income tax to withhold from a student's paycheck.

W-222 Minnesota Residency Form - Student employees who are legal residents Minnesota should complete this form. This authorizes Beloit College to exempt qualified students from the withholding of Wisconsin state income tax. The form may also be used to request reimbursement of Wisconsin state income tax previously withheld.

2010 Federal W-4 Form - A form which every student employee is required to complete so that Beloit College can withhold the correct federal income tax from their paychecks.
