Without a doubt, the financial aid process is information and paper intensive! In order to comply with federal and state regulations, we often have to request additional information from the student or parents. Or, perhaps, the family has asked that we consider unusual financial circumstances which the FAFSA information does not accurately reflect. Whatever the scenario, you can bet that there is a form involved.

This page contains links to those forms most commonly used in the financial aid process, as well as forms pertinent to student employment. Please note, however, that not all of these forms are required of every financial aid applicant. All financial aid applicants - whether prospective students or current students - must complete the FAFSA and the appropriate Beloit College Financial Aid Application below. If our office needs additional information to complete the financial aid analysis, we will inform the student, either by mail or email, and we will direct the student to the appropriate form, which may be downloaded here.

Forms Related to Financial Aid Applications:

Beloit College New Student Financial Aid Application - This form is required of all applicants for admission (first-year and transfer) who are also applying for financial aid.

Beloit College Continuing Student Financial Aid Application - This form is required of all current students who are applying for aid.

Dependent Student Verification Form - For a variety of reasons, a student's FAFSA may be selected for verification by the federal processor. The Student Aid Report - which a student receives after the FAFSA is processed - will clearly state whether the FAFSA is selected for verification. The Financial Aid Office will also inform the student that the FAFSA has been selected for verification and that additional information is necessary. Dependent students who are selected for verification must submit this verification form along with the required income tax materials.

Independent Student Verification Form - See the explanation above. This is the form to use if you filed the FAFSA as an independent student. Independent students who are selected for verification must submit this form along with the required income tax materials.

Revision Request Form - All families complete the FAFSA based upon a common income tax year. However, financial situations can change over time, and sometimes families feel that the FAFSA information and analysis based on that tax year does not accurately reflect their current financial situation. A number of factors can come into play: loss of job, change of employment, unusual medical expenses, etc. When a family requests that we consider unusual circumstances not reflected in the FAFSA, we ask them to complete this form and to provide documentation of those circumstances. The financial aid officer will use this information to determine whether the need analysis should be modified.
